Antibiotic-associated diarrhea (AAD) is a common and frequently distressing side effect of antibiotic therapy. It is estimated that up to 25% of all patients taking antibiotics will experience AAD during the course of their illness. Antibiotic-associated diarrhea typically occurs during the first week of therapy and is more common in children than in adults. Although most antibiotic-associated diarrhea involves no long-term complications, complications may include failure to thrive, malnutrition, bacteremia, sepsis, and increased antibiotic resistance. The pathogenesis of AAD is complex and poorly understood. Recent studies suggest that AAD results from an increase in bacterial colonization of the large intestine induced by an alteration of the normal intestinal flora. Exposure of the gut to antibiotics, usually ofloxacin, enhances the colonization of the gut flora. The mechanism(s) that enhance colonization in the presence of antibiotics is unknown. The onset of diarrhea follows a rapid phase of colonization during the initial days of antibiotic therapy and a slower phase of progressive diarrhea during the initial week of antibiotic therapy. In children, this process is often associated with poor growth, and the burden of disease is often underestimated. More than 80% of the antibiotics prescribed in children are unnecessary, and thus could be avoided. Thus, the increased colonization of the gut by resistant enteric bacteria, or vice versa, may be one of the causes of antibiotic-induced diarrhea in children. AAD has been observed with all types of antibiotics, although patients with beta-lactam antibiotic intolerance are more likely to develop AAD.
